Completed form is required before the dog may participate in training classes.
A copy of your dog's rabies immunization certificate provided by your veterinarian must accompany this 4-H form.
Record of Achievement
Complete 2 copies of this form. One copy to be placed in your green 4-H folder and the other copy to be turned in to the 4-H Department when entering projects.
